MRI Scan Cost in the US: With and Without Insurance (2026)

An MRI at a hospital can cost 5× more than the same scan at an independent imaging centre. This guide explains why — and exactly how to pay less, whether you have insurance or not.

Key insight: A knee MRI at a hospital in a major US city can cost $3,200. The same scan at an independent imaging centre two miles away: $650. The price gap is real — and avoidable.

US MRI Prices by Body Part (2026)

Imaging centre prices are at independent/freestanding facilities. Hospital prices are at hospital-based radiology departments. With-insurance figures assume deductible has been met.

With Insurance — What You Actually Pay

Having insurance doesn't mean the MRI is free — or even cheap. Understanding how US health insurance calculates your share is critical.

💳Deductible

The amount you pay out-of-pocket before insurance starts covering costs. Average US deductible: $1,735 (individual, 2024). If you haven't met your deductible, you pay the full negotiated rate.

Copay

A fixed amount you pay per service — e.g. $75 per specialist visit. Some plans have a copay for imaging ($150–$300). Applies after deductible is met.

%Coinsurance

Your percentage of the bill after deductible — typically 20% (insurance pays 80%). On a $2,000 MRI, your 20% share is $400.

🔒Out-of-pocket maximum

Once you hit this limit (~$8,700 individual, $17,400 family in 2024), insurance covers 100% for the rest of the year.

Surprise bill warning: If your deductible isn't met and you use a hospital, your insurer's negotiated rate (what you pay) can be higher than the cash-pay rate at an independent centre. Always compare the two before booking.

The Hospital vs Imaging Centre Price Gap

Example: Knee MRI — Same City, Same Quality

🏥 Hospital Radiology

$3,200

Facility fee + physician fee + radiology read fee — all billed separately

🏢 Independent Imaging Centre

$650

All-inclusive: scan + radiologist report

Action: When your doctor orders an MRI, ask: “Can you refer me to a freestanding imaging centre rather than the hospital radiology department?” Most doctors can do this — and many will if you ask. This one step can save $1,000–$3,000.

8 Cash Pay Strategies to Reduce Your MRI Cost

These strategies work whether you have insurance or not.

  1. Ask for a referral to a freestanding imaging centre

The most impactful step. Tell your doctor: 'Please refer me to an independent imaging centre, not the hospital radiology department.' This alone can save $1,000–$3,000.

  2. Call ahead and ask for the 'self-pay' or 'cash-pay' rate

Many imaging centres have a lower unadvertised price for self-paying patients. Say these words explicitly: 'I'm paying out-of-pocket. What is your cash-pay rate for a [body part] MRI?' You may get 30–60% off the standard rate.

  3. Use a price comparison tool

Sidecar Health and Healthcare Bluebook show real prices from providers in your area. Sesame Care lets you book MRI at transparent all-in prices ($200–$700 at many centres).

  4. Get a referral without insurance via Sesame Care

If you don't have a doctor or insurance, Sesame Care offers virtual consultations for $40–$60 — the doctor can issue a referral letter you can take to any independent imaging centre.

  5. Check for FQHCs and community health centres

Federally Qualified Health Centres (FQHCs) offer services on a sliding-fee scale based on income. Some offer or can arrange diagnostic imaging at subsidised rates.

  6. Ask about payment plans before you decline

Most imaging centres offer 0% interest instalment payment plans, often for 6–24 months. Never decline a necessary scan purely on cost without asking about financing.

  7. Compare cash-pay vs insured rate if deductible not met

If your annual deductible hasn't been met, you often pay the full insurer-negotiated rate — sometimes higher than the cash-pay rate at an imaging centre. Calculate both before booking.

  8. Apply for charity care or financial assistance

Most non-profit hospitals are legally required to offer charity care programmes. Ask the billing department about their financial assistance policy before or after your scan.

US MRI — Frequently Asked Questions

Without insurance, MRI cost ranges from $400 at an independent imaging centre to $8,000+ at a hospital for the same scan. The most common body parts (knee, brain, spine) typically run $400–$2,000 at freestanding centres. Always call and explicitly ask for the 'self-pay' or 'cash-pay' rate — many centres have an unlisted lower price for uninsured patients. Never accept the hospital list price without asking for a discount.
